What documents are needed to attach to the clinic?

To attach to the clinic, you need to collect a package of documents. It includes:

• Russian Federation passport (for children under 14 years of age — birth certificate) or other identity card;
• compulsory medical insurance policy;
• SNILS (if available);
• a document confirming the change of residence (upon relocation).

For persons subject to the Law on Refugees, one of the following documents will additionally be required::

• refugee ID card;
• certificate of consideration of the application for recognition as a refugee;

First of all, it is necessary to choose the medical institution to which the person wants to attach. In some cases, admission to the clinic may be refused. The following are the most common reasons.:

• Errors in the submitted application, incomplete package of documents;
