The surgeon talked about the risks of using fillers in the facial area
Filler injections have long been one of the most popular cosmetic procedures, but few people know or remember that the face is an area with a complex system of blood vessels. Sergey Karpovich, a plastic surgeon and rhinosurgeon, told Izvestia about this in an interview on October 24.
According to him, it is not the drug itself that is dangerous, but an inept cosmetologist with poor knowledge of anatomy and physiology. Therefore, if the filler gets into the lumen of the artery or strongly compresses the vessel, necrosis may begin.
"Not only tissue death can occur, but even blindness can occur. Fortunately, this rarely happens, only when the filler enters the central ocular artery through the nasal artery and through it into the retina of the eye," warns the doctor.
The specialist named the nose and the area between the eyebrows, where the arteries feeding the wings and the tip of the nose pass, as the most dangerous areas. He explained that if an injection fails, the skin first turns pale, then darkens and dies. As a result, a severe scar is formed, which can only be corrected by a series of reconstructive operations.
The periorbital zone, the area around the eyes, is considered no less risky. Here, the vessels are very superficially located, and the skin is thin. The penetration of filler into the vessels of this area also leads to necrosis of the eyelids and loss of vision.
"Even a microscopic amount of the drug can cause catastrophic changes. The patient is deprived of the opportunity to close his eyes completely, and this is already a threat to the cornea," the expert explains.
The central part of the forehead is also fraught with danger, Karpovich added. Deep injections in this area can result not only in skin necrosis, but also in an extremely rare but fatal complication — stroke. There are cases in the medical community when the filler entered the system of cerebral arteries through anastomoses.
The doctor assures that it is impossible to completely eliminate the risks, but they can be minimized if you trust the procedure only to professionals. An experienced cosmetologist knows the anatomy, works using blunt cannulas instead of sharp needles, and always checks the needle position before administering the drug.
The patient, in turn, can also play it safe. During the consultation, it is worth asking direct questions: what are the risks for this particular area, whether cannulas are used, whether there is an antidote in the office — the enzyme hyaluronidase, which is able to dissolve the filler in an emergency situation.
"A good specialist will not be offended by such questions. On the contrary, he will tell you in detail about all possible risks and security measures. A true professional always works with a full understanding of anatomy and respect for the main rule of medicine — do no harm," concluded Karpovich.
