A comprehensive health diagnostics project, the Moscow Checkup, has started in Moscow. It will allow citizens to calculate their biological age, determine the type of aging and receive personal recommendations on prolonging an active life. The average life expectancy in the capital has already exceeded 80 years, and now the city aims to give Muscovites up to 10 more years of high-quality, active and meaningful life. Moscow Mayor Sergei Sobyanin announced the launch of the project in June. For more information, see the Izvestia article.

From disease treatment to age management

Moscow Mayor Sergei Sobyanin announced in his blog the launch of a new project for comprehensive health diagnostics. According to him, in 2025, the average life expectancy in Moscow exceeded 80 years for the first time, which is seven years more than the national average.

— Healthy longevity is one of the most cherished dreams for each of us. Scientists all over the world come to the conclusion that health and a long active life depend on the person himself, his habits, decisions, and not least on the city in which he lives," Sobyanin said.

The city is moving from a reactive model of medicine — when a person treats an already existing disease — to a preventive one, when a doctor helps maintain health and quality of life for as long as possible.

— The city's task is to create all the conditions so that everyone can manage their age and get additional years of a busy life. To do this, it is important not only to treat diseases efficiently, but to identify risks even before symptoms appear. Therefore, within the framework of the philosophy of healthy longevity, we have launched the Moscow Checkout, a new health care program, 90% of which takes place online. This makes it convenient to integrate it into everyday life," said Anastasia Rakova, Deputy Mayor of Moscow for Social Development.

How the Moscow Checkout works

The new project of complex diagnostics was named "Moscow Checkout". It includes the provisions of the federal program of medicine for healthy longevity and an expanded set of laboratory and instrumental studies. Previously, many of them could only be completed for a fee in private clinics as part of prevention.

The initial stage takes place online, without the need to make an appointment with a doctor or visit a clinic. In the mobile application "EMIAS.INFO" a resident of the capital answers questionnaire questions and takes health tests. In addition, it is necessary to upload data from smart scales on body composition to the system or undergo bioimpedance in diagnostic complexes "Corners of health" in the centers "My documents". The system automatically processes the data and generates personal recommendations and referrals for an expanded set of surveys.

In addition, citizens will be able to calculate their biological age and understand which of the six possible scenarios is aging the body. .

"The data uploaded by the user is instantly processed by the system: it automatically calculates a person's biological age, determines an individual aging profile and generates a personal package of necessary in—depth examinations based on real risk factors," emphasizes Andrey Tyazhelnikov, chief freelance specialist in primary health care for the adult population of the capital.

AI agents at EMIAS analyze the results of questionnaires, functional tests, bioimpedance, as well as medical records and previous appointments, forming a single summary. If any abnormalities are detected, the doctor will conduct a telemedicine consultation directly on the smartphone.

The "heavy artillery" of preventive medicine

The expanded "Moscow Checkout" includes examinations that were previously available only in paid clinics. Among them are unique markers of aging, for which patients in commercial or foreign Anti-age centers pay impressive sums. Specialists check markers of latent inflammation, which allow them to detect tissue degradation at the cellular level before symptoms appear, and the level of oxidative stress. anti-age markers, reflecting the rate of aging of the body and the reserve of the adrenal glands, an ultra-sensitive marker of latent heart failure, genetic factors of vascular disasters, as well as a marker of bone destruction for the diagnosis of osteoporosis.

In addition to the laboratory base, the check—up includes a large-scale instrumental unit - from ultrasound of the vessels of the neck to bioimpedance measurement, which allows detecting age-related loss of muscle mass (sarcopenia).

— The health of the metropolis is now assessed comprehensively — through the prism of preserving the body's resources. — explains Natalia Shindryaeva.

Moscow is overtaking world capitals

For a long time, Singapore has been considered the benchmark of global preventive medicine thanks to the government's Healthier SG program. However, the Moscow Checkout brings the Russian capital to the world leaders in several key parameters.

In Singapore, the government program focuses on mass but standard screening: checking the level of normal cholesterol, blood glucose, body mass index and basic cancer screening. Moskovsky Checkout offers an expert-level anti-age screening. Bioimpedance measurement is used instead of surface BMI. Instead of basic cholesterol, there are genetic markers of vascular catastrophes. Instead of standard tests, there are markers of cellular aging and oxidative stress. Moscow offers for free what is available in Singapore only in elite private anti-age clinics for thousands of dollars.

"Moscow is removing all barriers: the basic steps, questionnaires and tests are conducted online from home, and the in—depth laboratory panel includes not just basic cholesterol, but genetic markers of heart attacks and markers of systemic inflammation, which the patient will not receive for free in the Singapore system," notes Andrey Tyazhelnikov.

Singapore actively uses the HealthHub app, but prescribing examinations and interpreting the results mostly require an in-person visit to a family doctor. Moscow has one of the most powerful digital medical ecosystems in the world — EMIAS. The primary stage of the checkout is fully digitized: questionnaires, data collection from home gadgets, automatic routing and calculation of biological age occur online. Artificial intelligence, trained on advanced clinical experience, generates a risk profile without the participation of a doctor at the first stage, which ideally meets the needs of active citizens.

In addition, in Moscow, longevity is seen as a complex result of environmental management. Advanced medicine, "Health corners" in the MFC, parks, accessible sports and cultural infrastructure are within walking distance for every resident.

Urban environment as a factor of longevity

The philosophy of healthy longevity in Moscow goes far beyond polyclinics. As Sergei Sobyanin emphasized, "medicine, culture, sports, roads, parks, and various institutions — the entire urban environment should create conditions so that every Muscovite can enjoy more than ten years of active and fulfilling life."

High-quality sports complexes and outdoor playgrounds with exercise equipment appear near each house. The Moscow Longevity project is in every district, the city offers residents cultural and leisure projects. Diesel buses are being replaced by electric buses and other environmentally friendly vehicles. Parks, embankments, bicycle infrastructure — all this works for one purpose.

— The entire infrastructure of the megalopolis works as a single mechanism to increase the human life resource, — summarizes Natalia Shindryaeva.

Gamification and Engagement

Biological age, the profile of aging, and visualization of progress create an effect of engagement and motivation. Taking care of your health is becoming similar to working with a personal tracker: the user sees changes in indicators and feels the result. Gamification is an additional driver: mobile apps, wearable devices, and activity trackers are changing user behavior, and people are beginning to perceive health as a system of achievement and personal progress.

In many countries, the "care anywhere" model is already being formed — medical services integrated into a person's daily life. Prevention, personalization, AI, and gamification combine into a single ecosystem where health and age management cease to be an episodic event and become an ongoing process. Moscow ranks among the largest megacities and is ahead of them in many ways, making the philosophy of healthy longevity widespread and familiar to millions of residents.
