After 40 years of age, classical plastic surgery techniques may not give the expected effect and even visually age the face. This was announced on November 29 by rhinosurgeon and plastic surgeon Sergey Karpovich Gazeta.Ru».
"To simplify, a young face looks like a thick mattress with elastic springs, and after 40 the springs sag and the upholstery stretches. And if you just pull on the upholstery, as with the old lifting techniques, the effect will be short—lived and unnatural," the expert said.
Karpovich noted that with age, the production of collagen and elastin decreases, microcirculation deteriorates, the distribution of adipose tissue changes, and bone loss is observed. He stressed that the old approaches, focused only on skin tension, create a mask effect and do not preserve natural facial expressions.
The specialist recommends modern methods of deep lifting with SMAS and muscle skeleton work, as well as careful distribution of adipose tissue during blepharoplasty for patients over 40 years of age. In liposuction of age-related patients, he advises using laser and radiofrequency technologies to stimulate collagen.
Rhinoplasty also requires a reconstructive approach: strengthening supporting structures and working with cartilage grafts. Karpovich emphasized that age is not a contraindication to plastic surgery, but the intervention must be individually planned and performed by an experienced surgeon.
The specialist identified several universal rules: the priority of reconstruction over "cutting off excess", an integrated approach to correcting several zones, restoring volumes with your own fat or fillers, and using gentle technologies.
"The main mistake is to go for a boilerplate operation that would suit someone at 25. After 40, you need an individual plan based on diagnosis and understanding exactly how your tissues respond to the intervention," concluded Karpovich.
