A condition in which a person's pupils vary in size is called anisocoria. Usually, this phenomenon is not an independent disease, but serves as a symptom of other ophthalmological, neurological, or somatic disorders. Anna Solovey, an ophthalmologist at the 3Z clinic, told Izvestia about this on August 29.
"This condition occurs due to improper muscle function, constricting and dilating the pupil. A deviation is considered to be a difference in pupil size of more than 1 mm, and more often only one eye is affected," the doctor clarified.
The specialist identified several types of anisocoria. Physiological anisocoria, in which the size of the pupils varies slightly, is due to genetic characteristics and does not require treatment. It occurs in about 20% of people. The pathological form, with a difference of more than 1 mm, can develop due to other eye diseases, brain lesions such as neurosyphilis, tick-borne encephalitis or tumor processes, as well as diseases of internal organs.
A common cause is Horner's syndrome, which manifests itself against the background of a stroke. Mechanical anisocoria is associated with iris injuries, postoperative complications, or inflammatory eye diseases. The pharmacological type is singled out separately, which develops due to the action of local medications and usually resolves on its own.
"Even a slight difference in pupil size can cause serious visual discomfort, as it disrupts the process of light perception. In a bright environment, an eye with an enlarged pupil receives an excess of light, while in the dark, on the contrary, it suffers from a lack of it. This leads to difficulties with orientation at dusk," said Solovey.
Patients often instinctively cover the eyelid of the affected eye, which can provoke the development of false ptosis and progressive visual impairment. The treatment strategy for anisocoria depends entirely on its root cause and is determined by a specialist after a comprehensive diagnosis, including anamnesis collection and a number of studies. Timely and well-prescribed treatment provides the best chance of a favorable outcome and preservation of vision, the ophthalmologist concluded.
