Converting budget places into targeted ones looks like a well-founded step. The state invests significant resources in the training of future doctors, and one would expect students to be responsible in return. Kirill Masliev, Candidate of Medical Sciences, member of the State Council Commission on Active Long-term Life, Head of the Department of Preventive Medicine at the Rudn University, founder of the Heroes Charitable Foundation, told Izvestia on August 27.

"For applicants, this means a more balanced choice of profession and an informed attitude to learning: now their training can be linked to specific obligations to the healthcare system. This can increase discipline and strengthen mutual responsibility between the state and the student," he said.

In a number of regions, there remains a noticeable shortage of primary-level specialists, and targeted personnel allocation could make it possible to close such gaps point-by-point. In addition, the share of paid recruitment is decreasing, which means that government control over the quality and quantity of future doctors is increasing. The availability of contracts and penalties for non-fulfillment of obligations could discipline students and reduce the risk of staff leakage.

According to Masliyev, this decision may have a positive effect on the healthcare system. Government agencies, especially in the regions, can receive a guaranteed influx of specialists. Young doctors will take the profession with greater responsibility, realizing that there is an obligation to the state and patients for their training. As a result, the primary link can be strengthened, the overload of existing employees can be reduced, and the availability of assistance can increase.

In the future, such a model can create a more stable and predictable staffing system.

"But such a distribution can play in the other direction. For example, certain regions will receive more specialists in one or another field of medicine, while other regions may experience shortages. Therefore, this system will need regulation and control," the expert shared.

He is also confident that it is necessary to save some budget places, even in the case of the introduction of this system. Since it is not very advisable to introduce a distribution system only.

On August 26, it was reported that the Ministry of Health of the Russian Federation proposes to make all budget places in the main programs of higher medical education targeted. The Ministry also proposed to oblige target doctors to pay compensation in three times the amount for refusing to complete training.
