Prostate adenoma remains one of the most common urological pathologies in men after 45 years of age. And if in middle age the disease is detected in 40% of cases, then by the age of 80, 90% of the male population worldwide has adenoma. On June 20, Vladimir Besprozvanny, a urologist and andrologist at SM Clinic, told Izvestia whether there is a relationship between this disease and prostate cancer.
Prostate adenoma, or, scientifically, benign prostatic hyperplasia, is an overgrowth of the glandular tissue of the male reproductive organ. The older a man gets, the more intense the growth of abnormal tissue and the larger the neoplasm itself.
According to the urologist, millions of modern men live with prostate adenoma, unaware that their health is not all right. Blurred symptoms of the onset of the disease are often to blame for this: frequent urge to urinate, including at night, is easily confused with prostatitis. Over time, the dynamics of the disease only increases. Men experience a feeling of incomplete emptying of the bladder, a weak stream when urinating, involuntary leakage of urine, and in later stages— acute urinary retention, which requires urgent medical attention.
"As the adenoma increases in volume, it begins to put more pressure on the urethra, which makes urination much more difficult. There is a risk of dilation of the ureters, stretching of the bladder and its complete dysfunction. As a result, the sick person can no longer empty himself," said Bezprozvanny.
In addition, against this background, kidney failure may develop, which manifests itself in the form of severe thirst and dry mouth, as well as sharp pain in the lower abdomen, gradually dulling.
The doctor also denied the widespread rumors associated with a high risk of degeneration of adenoma into prostate cancer. In his opinion, such arguments have no evidence-based clinical basis. Adenoma and prostate cancer are completely different diseases, with their own specific development. But what you really need to remember is that the adenoma is constantly progressing, and this is a dangerous moment that threatens serious complications in the future.
One of them is hydronephrosis. This disease occurs due to a violation of the outflow of urine, as a result of which the calyx-pelvis system of the kidneys expands, and their tissues undergo atrophy. In this condition, only surgery will help a sick person to save vital organs.
"New technologies have now appeared in urology and andrology that allow doctors to perform operations with minimal injury to prostate tissue. Transurethral resection (TUR of the prostate) and Rezum water—steam therapy are among the most gentle methods of adenoma removal. Both technologies are very effective: patients achieve full recovery within a short period of time," Besprozvanny added.
However, Rezum also completely minimizes the risks of retrograde ejaculation (the movement of sperm from the urethra into the bladder), which often develops after urological surgery. Therefore, the doctor advised to choose those techniques that will help not only to cope with the problem, but also to improve the quality of life in the future, without any consequences.
He stressed the importance of regular prevention in the case of the prostate gland. After all, this organ is a "second heart" for men and it is very important to take care of his health.
Earlier, on June 17, Alexander Sysoev, oncourologist at SM Clinic, told Izvestia about an analysis that helps detect prostate cancer. According to him, a blood test for prostate-specific antigen helps to detect a dangerous disease at an early stage, when the tumor has not yet metastasized to neighboring organs.
