The pilot of a Black Hawk military helicopter, which collided with an American Airlines passenger plane, ignored warnings about lowering altitude and turning before the crash. This was reported by The News York Times (NYT) on April 28.

"In the last seconds before the crash, his pilot (helicopter. — Ed.) did not comply with the instructions of the co-pilot, an army flight instructor, to change course," the article says.

It is noted that radio communications, among other things, failed, as a result of which some of the dispatcher's instructions were cut off when the crew pressed the communication microphone. According to the NYT, the pilots might not have heard important information.

An American Airlines plane and a Black Hawk helicopter collided on the night of January 30 near Ronald Reagan National Airport in Washington, after the accident they fell into the Potomac River. There were 60 passengers and four crew members on board the civilian aircraft, and three military personnel on the helicopter. US President Donald Trump announced the absence of survivors of the plane crash.

On February 1, it became known about the recovery of the bodies of three crew members of a military helicopter. Among the military personnel on board the Black Hawk were Sergeant Ryan Austin O'Hara, Petty Officer Andrew Loyd Eves and Captain Rebecca Lobach. Rescuers found and retrieved the remains of the bodies of all those who died in the plane crash on February 5 when removing the tail and nose parts of the aircraft from the bottom of the Potomac River. All 67 victims were identified.
