The first fully Russian biopolymer implants are being tested at Sechenov University. According to their characteristics, such materials are close to human bone, which makes their use more comfortable for humans, the developers said. In the future, personalized prostheses will be manufactured on the basis of domestic polymers within a few days. The Center for Engineering Development (CIR) of the I.M. Sechenov First Moscow State Medical University has created implants for reconstructive and plastic surgery in the maxillofacial region made entirely of Russian medical polymers. Previously, there were no domestic medical polymers on the Russian market.

The project to create implants from domestic polymers is being implemented jointly with the Department of Maxillofacial Surgery named after Academician N.N. Bazhanov of the Borovsky Institute of Dentistry of Sechenov University, Alexander Kulish, Director of Technology Commercialization at the First Moscow State Medical University, MD, told Izvestia.

At the request of clinicians, the designers designed a 3D model of an implant for reconstructive surgery on the lower jaw based on a CT scan of a real patient. At the same time, domestic software was used. Then, several samples of medical products were made on a 3D printer and injection molding machine, using two different biocompatible polymer materials — polysulfone and polyesteresterketone. Biocompatible materials were synthesized at the Center for Advanced Materials and Additive Technologies of Kabardino-Balkarian State University.

— For the manufacture of implants, we used Russian polymers, which were not previously available on the market, — said Alexander Kulish. — To date, there are no foreign polymers of decent quality for medical use on the market.

Unlike metal implants, the developed biopolymer is 40% lighter than titanium, in addition, it has a number of advantages, it does not cause allergies, and its characteristics are closer to human bone, which makes the use of such implants more comfortable for humans, said Alexander Kulish. It does not corrode or hydrolyze, is highly resistant to degradation in the biological environment, is capable of sterilization by any method, and is fully biocompatible and safe at the cellular and genetic levels, the doctor emphasized.

Import substitution in implantology

The first implants made of domestic polymers were produced at the experimental industrial sites of the engineering center. They are currently being tested at the university's clinical center to obtain a registration certificate.

The researchers will have to determine which polymers are optimally used for maxillofacial surgery, as well as work out the technology for manufacturing implants. In addition, the engineering center is developing biodegradable polylactide implants for pediatric neurosurgery and plastic surgery, as well as for traumatic interventions in the maxillofacial region. By the end of this year, the University plans to create a center for individual prosthetics. By order of the clinicians, personalized implants will be made here literally at the patient's bedside within a few days.

Many foreign companies that produce medical products for reconstructive, plastic and maxillofacial surgery have now withdrawn from the Russian market. And the demand for such products in our country is very high," Professor Sergey Ivanov, Head of the Department of Maxillofacial Surgery named after Academician N.N. Biopolymer—based implants are one of the trends in modern dentistry, said Vyacheslav Minko, Chief Physician of Dr.Minko Dental Clinic.

— They have many advantages, for example, hypoallergenic, similarity to the bone structure of the jaw, flexibility, durability, — said the specialist.

Facial implants are used to recover from severe injuries with significant destruction of the bones of the facial skeleton and skull, after removal of large tumors in the bones, to compensate for the occurring birth defects. Fortunately, these are not such frequent phenomena, but people who encounter this in their lives experience serious suffering, said Andrey Ilyin, a maxillofacial surgeon and commercial director of Intel Medical Group. According to him, the use of biopolymers for implantation is a promising area.

— Regarding possible disadvantages, they may appear at any of the mentioned stages of work, it is important that all components of implant production are of high quality and convenient. All this is checked at the stages of mandatory pre—clinical and clinical trials," the expert emphasized.

The advantage of any implant is that no additional injury is inflicted on the patient, that is, there is no need to remove the graft (a fragment of a rib or other bone), said Sergey Rozhnov, a dental surgeon, implantologist, and chief physician of the Implant Time Dental Implantology center. According to him, rehabilitation of such a patient is easier, and artificial materials are the future. In addition, they should eliminate the shortcomings and complications that arise when using titanium implants.
